The University of British Columbia School of Health and Exercise Sciences is committed to providing students with lab course offerings that are informative, enriching and safe. However, there are risks inherent in the lab environment and in the performance of physiological, biomechanical, and medical testing, both as an investigator and a subject
I certify that there are no health-related reasons or problems that preclude or restrict my participation in these Courses. If you are unable to make this certification, you are not necessarily precluded from taking these Courses. Please consult with the Course Instructor or the Laboratory Coordinator.
For the Tests, the risks include, but are not limited to: potential exposure to biohazards such as saliva, vomit and/or blood; bruising and/or infection due to finger pricking; muscle cramping; muscle soreness; strained or injured muscles; light-headedness and fainting; nausea and vomiting; mild headache; epileptic fit due to transcranial magnetic stimulation. I acknowledge these risks and accept them as an inherent part of the Courses. I understand that participating in the tests is strictly voluntary and will not affect my grade for any of these Courses. I understand that if I volunteer to undergo a test, I am accepting these risks. If I have health-related reasons or problems that preclude or restrict my participation in a test, I will not volunteer to participate in that Test.
I understand that no part of any of these Courses, including the Tests, is for diagnostic purposes. A Physician will not review the results.
I will follow all safety and other instructions provided by the Course Instructor and those administering the Tests, share in the responsibility for my own safety, and not endanger others. I will immediately report all unsafe acts, dangerous conditions and injuries during lab to the Teaching Assistant, Laboratory Coordinator and/or Course Instructor.
I understand that an emergency may develop which necessitates the administration of medical care. Therefore, in the event of injury or illness, I authorize the University of British Columbia to secure any appropriate treatment on my behalf.
